
Community nurses are registered nurses who work in schools, local surgeries, health centres and also visit people in their homes. They provide health care and help and advice on health matters. This is equivalent to the term District Nurse in the past.
When a patient leave's hospital, arrangements may be made for a community nurse to visit them and continue their care at home. GPs can also refer people to the community nursing service.
Whilst Dorothy House provides nurses and carers (from our base in Winsley) who work in the local community, they are different to District/Community Nurses.
