NHS continuing health care is a package of care for people with ongoing health care needs provided outside hospital but arranged and funded by the NHS. It can be provided at home or in a care home.
People who are eligible are likely to:
have a complex medical condition that requires a lot of care and support
need highly specialised nursing support
Someone nearing the end of their life may also be eligible if they have a condition that is rapidly getting worse and may be terminal.
Eligibility is determined by assessment. An initial screening by a health or social care professional is followed by a full assessment by a multi-disciplinary team.
